Diabetes is the seventh (7th) leading cause of death in the U.S. and fifth (5th) in the District. Wards seven (7) and eight (8) are amongst the highest rates in the nation.

At United Medical Center, we focus on improving the health and the delivery of care to persons with diabetes in our community. The operative premise is a commitment to reduce health disparities where the most common chronic diseases such as diabetes affect our underserved minority communities. Our program is certified by the American Diabetes Association meeting national standards for Diabetes Self-Management Education. We provide education for inpatients and outpatients and utilize community outreach, interactive diabetic disease workshops to restore and/or stabilize the health of our diabetic clients. The program has been very effective in helping patients change their lifestyle by taking an active role in their care. Program success is also apparent through elimination of barriers to care and a special focus on medication adherence. Through consistent work Physicians, Nurses and Pharmacists are responsible for the execution of best practice standards yielding favorable outcomes and subsequently improving the quality of life for diabetics who come to us for care and actively execute the treatment regime.
